Provider Directory

Find a certifying provider
in North Dakota.

North Dakota doesn't maintain a public registry of participating providers. We've put together the known options — starting with the simplest path.

Who can certify you? Any licensed North Dakota physician (MD/DO), physician assistant (PA), or advanced practice registered nurse (APRN) can certify a qualifying condition — as long as you have an established patient-provider relationship with them. As of August 1, 2025, telehealth is now permitted for initial certification visits.

Are you a North Dakota veteran using the VA Healthcare System?

You have a completely different — and simpler — pathway. VA veterans don't need a provider appointment. Your VA treatment records replace the provider certification entirely. No clinic visit required.
